I need help recreating a Wine Order Form

I just need some help for the quickest way to recreate the form shown below. It's a Wine order form, no payment processing for now but I would like it to calculate the $ amounts for each wine selected and provide a total. I'm new so this is proving to be very time consuming. I'm hoping there's a way to copy and paste a product line with fields for the Product Name, Price, Qty, Extended price so that I don't have to create each line separately.

The form will be embedded on the new mobile-friendly site. It will need to generate an email to my client and they will process manually from there.

It's using our Purchase Order integration which allows you to set up the product with options, but no payment involve. The email template is also created for you.
