Could you try to check another network to test the form? Also, you can try another browser to see if the issue is related to the used browser. I have cleared your form Cache to refresh the data. You can try to test the form again.
If you still encounter the same issue, you can share with us the used device and its operating system. Also, you can share the used browser for further checking.
