Hi cb88RG,
Thanks for reaching out to Jotform Support. I understand that you're looking for a way to pre-populate your form with data from your spreadsheet. You can easily do this by adding the Spreadsheet to Form Widget to your form. Let me show you how:
1. In Form Builder, click on Add Form Element on the left side of the page.
2. Under the Widgets tab at the top of the Form Elements menu, search for the Spreadsheet to Form widget.
3. Click on the Spreadsheet to Form widget to add it to your form.
Now, let's set up the Spreadsheet to Form widget:
1. Select the Spreadsheet to Form widget field and click on the Wand icon that comes up on the right side of the widget field.
2. Click on the Upload File field under the General tab of the Widget Settings menu to add your spreadsheet.
3. From the Dropdown menu under Access Code Column, choose the column in your spreadsheet that contains the access codes.
4. Then, click on Update Widget at the bottom right corner of the Widget Settings menu.
Check out the screencast below to see my result:

Give it a try and let us know if you need any other help.
