Why can't I enter my Jotform account?

    A doctor's office used Jotform to fill out paperwork. Apparently it created an account for me. Now, however, I can't create a form for myself when I sign in.

    Attached are screenshots.

    • CREATE FORM

    • Start from Scratch

    • Classic Form

    • Form not found

    Am I limited because my email is somehow connected to that doctor's office? I would like to get started with the free Starter package so I can determine if Jotform will work for a different business.

    Please advise. Thanks.
