Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got two forms having fields that violated our Terms of Use. The one named "Card Inf" has fields for collecting Credit Card details in plain text fields:
In line with our PCI certification, we have to deny the collection of credit card details on regular forms. But, we have other PCI-compliant ways you can do this. All you need to do is choose one of the payment processors available on our platform and integrate it into your form. You still won't be able to collect credit card details directly, though. If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of our Payment Authorization feature.
If you want to collect payments on your form, you can use any of our Payment Integrations. Let me show you how to add a payment gateway to your form:
- In Form Builder, click on Add Form Element in the upper-left corner of the page.
- Go to the Payments tab, and choose a payment gateway.
- Connect your form to the payment processor by clicking on the Connect button on the Payment Settings panel that pops up.
- Once done, click on Continue in the bottom-right corner.
Aside from the form mentioned above, you also have a form named "Account Info" have fields for collecting Login Credentials:
Note that this is also against our Terms of Use. Since both forms got zero submissions yet, I've removed the offending fields from the forms, and reactivated your account. You should be able to log in again now.

Hi barbod,
For collecting credit card details, you can refer back to my reply above on how to collect payments using your form. But for collecting passwords, this is strictly not allowed through Jotform forms, as stated in our Terms of Use:
If you add fields for collecting passwords again, I'm afraid your account will be suspended again once our Phishing Detecting System detects them.
