Downloading Fillable .pdf forms, Fields are getting jumbled and out of place

Some of the fields are not downloading properly. Some field text isn't even loading (the sub field text). Some fields are showing up at the bottom or randomly placed throughout the form when trying to download the fillable .pdf.

Thanks for reaching out to Jotform Support. I'll do my best to help you. I've checked out the screenshot you shared with us. I've cleared your form caches; your forms are now refreshed. I cloned your form Supplemental Needs Trust Questionnaire and downloaded the fillable PDF of it to see if I could replicate the issue, and I noted that only area the multiple choice option was not in alignment. So, to resolve an issue, I added the empty box and everything was aligned. Let me share the screenshots below and after.

1. Open your form in the PDF editor under the build section, Click on the Add form elements.
2. Choose an empty space. That's it.
