I see you have three fields connected to the MailChimp integration. I checked your submissions. Since you have the same email Id in all the 5 submissions so it should update the existing row created in the MailChimp.
Are you not receiving the submission data at all?

Also, please make sure that your API key matches your mail chimp API key. Please log in to this page https://us17.admin.mailchimp.com/account/api/ to check your API key.
