    Hello! I was wondering how to add the file name for images added to survey on the PDF report once the survey is completed?     Hi RobertsCourtney,

    Thank you for reaching out to Jotform Support. You can use conditional logic to retrieve the uploaded file name and store it into a field. You can hide this field so that it will only be shown once you download the PDF copy of the submission. Let me show you how:

    1. Add a short text field and label it as File Name.

    a. In Form Builder, click Add Form Element on the left-hand side of the page.

    b. Drag and drop the short text field into the form.

    c. Click the gear icon to get to the field properties.

    d. Go to the Advanced tab and toggle the slider to ON for Hide Field.

    a. In Form Builder, on the orange navigation bar, click Settings

    b. Go to Conditions tab and click Add New Condition

    c. Configure the conditions like the ones below.

    d. Click Save to apply the changes.

    Hello, so it's not giving me the same drop down options like yours when I go to select the file. It doesnt give me target or value options :/

    So I went in and made all of the changes to page 1, but the links arent showing up correctly, as they should under the image. It randomly shows 2 of them however at the end of final page https://form.jotform.com/242466080841153

    Hi Courtney,

    I cloned your form and tested it to see if I could replicate the issue, and I ran into the same issue. After investigating it a bit, I found that some of the File Upload widgets don't have a file name field below them. Check out the screencast below:

    As you can see, there's no file name field below those File Upload widgets. To fix the issue, we need to add a Short Text widget below each File Upload widget and use conditional logic to copy the name of your file to that field by following the steps provided by my colleague, Mafe_M.

    Hi Courtney,

    I saw that only the first three file name fields have a conditional logic to copy the name of the file. The other file name fields don't have a conditional logic, so they're being submitted empty, and empty fields will not appear on your PDF document. Check out the screencast below:

    As you can see, after the first three file name fields, the rest of the file name fields don't have a conditional logic to copy the name of the file. To fix the issue, we need to add a conditional logic to those file name fields by following the steps provided by my colleague, Mafe_M.

    So I added in the conditions to all of the file names on page 2, but it's still not saving with the file image name on the PDF. It does it still randomly for 1 image at the bottom of the report.

    Hi Courtney,

    Thanks for waiting. I can confirm that the File Name fields and conditions to pass the file name's value to it are properly set up. However, the file names are not collected because your form is set to clear the value of hidden fields. To fix this, please change your form settings and set it to not clear the value of hidden fields. Here's how:

    1. In Form Builder, click on Settings on the top of the page.
    2. Click Show More Options.
    3. Scroll down and select Don't Clear under Clear Hidden Field Values.

    Also, I checked the PDF Editor and I see that the File Name (Short Text) elements are not added to the PDF template. Please go to the PDF Editor and add the hidden File Name fields below each of the File Upload elements. Here's how:

    1. In the PDF Editor, click on the Add Element button.
    2. Go to the Form Fields tab.
    3. Find the File Name fields and drag it below the File Upload field.
    4. Repeat the same steps for all the File Name and File Upload fields.

    So that did help but it's not displaying 100% correct-

    1. It now shoes the file name short text when it's supposed to be hidden per the forms current settings
    2. It should show the file name right under each image (and the file name should be clickable so the image pops up a clearer view once the file name is clicked). Here's that it looks like as of now via PDF with 3 images uploaded in one upload space

    Hi Courtney,

    If there are multiple file uploads in a single file upload field, then the file names will be displayed one after another in the short text field. Also, there's already an option in the table to download the attachments in bulk, for a specific form, see the screenshot below.

    If you want to get the links to the images uploaded, you can integrate with Google Drive and/or Google Sheets.

    I dont think I asked the question correctly. I'm looking to have the hyperlinked jpeg name (so when you click the name, you can see image better ), either right above or even below each image on the PDF format. As of now, the file name is not hyperlinked and it's not listed, and you can't tell which file name goes with which file when several are uploaded at once.

    Hi Courtney,

    If you need to add the file name at the bottom of the File Upload field but make the file name a hyperlink that downloads the submission file in the PDF report, you can do the workaround below:

    1. Add the Paragraph element to your form.
    2. Add the File Upload field's unique name in it. It will capture the file name. Here's the guide on How to Find Field IDs and Unique Names, How to Add File Name for Images Uploaded on PDF Copy of Completed Survey Image 1 Screenshot 40
    3. Highlight the unique name.
    4. Click on the Hyperlink tool.
    5. Paste this to the URL box: https://jotform.com/server.php?action=getSubmissionPDF&sid={id}&formID=242525609655058

    After those steps, do a new submission, download its PDF report, and you'll see that the file names are hyperlinked. Clicking it will download the submission file.

    Hi Courtney,

    I believe some of the File Upload elements on your PDF Editor have the image link enabled. This is why some images have a hyperlink below the images. Here's how you can enable that on the other File Upload fields on your PDF Editor:

    1. In the PDF Editor, click on the Gear icon next to the File Upload field.
    2. Check the box beside the Show image link.

    Please note that you need to do those steps on every File Upload element.
