How can I create a complex table in a form?

Hello,

I would like to create an online booking form for our Summer Holiday club. The hard copy booking form is attached, this is what I am trying to replicate. The link I have included is where I have got to with my form, it is much too complicated in my opinion, for a new visitor to fill in correctly.

Can you advise if there is a better way of approaching this problem?

I think that only the Input table looks a bit complex. The other parts of the form are fine. You can use the Date Picker widget and let the user to choose the week, date and time. Perhaps, you can use radio buttons and ask the user to choose the activities and the other options. So, you can try to replace the the Input table with some date widget and replace the other fields with Single Choice, Dropdown or Multiple choice fields.
