Yes, I see you have posted the request using a guest account. Here's the thread: https://www.jotform.com/answers/1859386.
The form seems to be in a PDF format. We allow PDF import, so you may want to try that.
Go to My Forms page (https://www.jotform.com/myforms).
Click Create Form button
Choose your Form Layout
In the next window, click the Import Form option:
Choose Import PDF option and click Continue button:
In the next window, you will be presented an option to upload the PDF file. Select your PDF form to upload:
You only need to follow the next processes, and that's it! There are previews in the page. The settings are similar to what you see in the regular form including the form email, conditions, integrations.
Note that this new PDF feature does not allow you to add fields in the form. The import tool automatically detects the fields and add them in your form. There are elements available, but they're limited to the heading, submit button, text, divider, section collapse, and page break.
