Upon review of your account it was found your form: http://www.jotform.us/form/91428371611151 which is suggesting to use an specific credit card for charges, was flagged for phishing activity. No part of credit card number can be requested or offered. The system will recognize them as in violation of terms and will autosuspend the account.
We have reactivated the account but you have to remove or reword the query in order to avoid further inconveniences.

Indeed the form "Credit Card Authorization Form" triggered our Phishing Detector Tool causing the autosuspension of your account, as my colleague previously mentioned: no part of credit card number can be requested or offered. The system will recognize them as in violation of terms and will autosuspend the account.
Your account has been reactivated, kindly modify it in order to avoid another autosuspension.
