I just found out about this widget today (through your email) and decided it might be a good fit for a form I'm creating. I added it to the form and made several tests.
It doesn't seem to work in your mobile app - while it offers the camera app as a choice, it won't activate it. It does allow selecting an image form the library (previously taken), but that will not do for this form.
If I access the form via a mobile browser, it seems to work as expected (though it appears I still have to select a camera, despite setting the widget to default to Rear Camera).
When the form is submitted, the image will not show in the email (shows as "broken", but opens in a browser window if clicked). It also doesn't show in the App except as a blank space. I can view it logging into the website and also saving a submission as PDF.

One more thing...
In your App, when viewing a form submission, there is a banner at the bottom that states "For more results please connect to the internet" - when I am connected 100% both WiFi and Data!

I cloned your form and made several tests on my Samsung Galaxy A5 but I am unable to replicate the issues you reported. The camera is working fine on my end. Please check if the camera is blocked in your browser.
The first widget on your form is set to load Back camera.
The image is also not broken in Email Notification on my end.
Is your form embedded? If so, please provide us a webpage URL.
Also, I am not sure which app are you using. Our Mobile App is used to view submissions.
Can you please tell us where exactly was this screenshot taken?

App - which is where that last screenshot is from that says "connect to
the internet".
It seems I was using the JotForm Next app (which allows filling the app
and better management of it) - I removed it and installed the "regular"
JotForm app...this is worse. It ONLY allows for viewing the submissions,
the form fields are not in order (shows all date fields, then all
comment fields...etc.) AND the photos still don't show, now there is an
extra long URL instead.
I reinstalled JotForm Next and the submissions look fine (except for the
same blank image issue). Also, despite creating a username/password, I
can't login to JotForm Next with it - though Google login works.I filled in the form through a browser on my PC (not ideally how it will
be used). The images are still not showing in my email - neither on
mobile (GMail) nor PC (Mozilla Thunderbird or directly in GMail). (see
attached)
Same applies to filling int in on a mobile browser (though oddly, the
form asked for permission - and worked - on Firefox Mobil but not Opera,
which is Chrome based).
Very frustrating. It seems viewing through the browser is the only
failsafe way to see submissions with photos.

We would like to apologize for any inconvenience. As noted in the Google Play Store, the JotForm Next App is still in production and may have issues that still need fixing.
I was able to reproduce the issue you described with the Camera option of the Take Photo widget when being accessed within the JotForm Next app.
And, we would greatly appreciate if you let our developers know about the issue(s) you found by sending your feedback within the app.
As for your other concern where the picture that was taken from the Take Photo widget not showing on email notification sent to you when your form is submitted, I have moved it to a new thread considering it is about another topic.
