We are currently using Gravity forms but want to switch over to JOT forms. We have a form that a person submits information to a particular member and the rule is created to send that information to member selected. Can we upload existing database (Name And email) to Jot forms or do we have to recreate?

If you want to create the form that is available in the URL that you have shared above, you can import it into JotForm. The guide below will assist you here. While selecting the form layout, you can select the Classic form type.

Now, if you have a form as a fillable PDF, it can also be imported into JotForm.
