I believe that you are referring to the form 81454457359163 in your account. As I check the integration, it seems to be set correctly. However, I notice that there is a special character (#) being used in the field labels. Could you try removing this special character from the labels and see if that works?
If the issue still persists, please check if the integration works with minimum number of fields to isolate the issue further.

Still not working, I wish you can login into my system and show why it is working on your side and not mine or if you can fix it for me.
I don't mind providing my airtable login info.

I have tested the AirTable integration by cloning your JotForm to my account. When I send a test submission, the AirTable integration is working fine. We request you to try the integration with Name and Email fields only and see if that works? You may try adding the other fields in the integration later to identify the issue further.
