Thanks for reaching out to Jotform Support. I tested your form and found issues with your conditions. We can assist you in properly configuring them so that the fields that are expected to appear based on the type of claim. Can you tell us if a user can select multiple or just one claim?

They should only be able to select one form. I'm assuming I should have used a radio button instead of multi box right?

Hi Zhou,
Yes, however we can still limit the option of the multiple choice field to one. But I reviewed your form and there are 8 options in the type of claim, but only 5 details pages are on the form. Are there options for the type of claim where they will see the same page? Can you provide all of the pages needed for each type of claim? You can simply provide the page number, and we will setup a cloned form for you to view the results.
Once we hear back from you, we'll be able to move forward with a solution.

I've limited the options. Homeowners and Dwelling share the same page.

Hi Zhou,
It's easy to accomplish what you like to do. Let me show you how:
1. First, we must remove the pages 3 to 6. Simply hover your mouse over each page and click on Remove page.
2. Then we remove the other submit buttons, leaving only the last submit button.
3. Add a Show/Hide Field condition to show the fields based on the user's answer in Type of Claim field.
a. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
b. In the panel on the left side of the screen, select Conditions.
c. Select Show/Hide Field and then set your conditions just like you see below.
d. Then, click on Save and you’re done.4. In addition, remove any existing conditions from your form to avoid conflicts with the new conditions I've set up.
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- In the panel on the left side of the screen, select Conditions.
- Click on the old conditions or Select All if you have not yet added the new conditions, and then click Delete.
5, You must also set a maximum entry limit of one in the type of claim field so that they cannot select more than one option.
a. Select your element's Gear icon to open its properties.
b. Go to Surveying tab.
c. Set the Minimum to 1 and Maximum to 1.
