Change the icon of the shortcut link on iPhone

I copied the link of the module and pasted it on the desktop of my phone (see attachment), is it possible to change the icon?

If that's not possible then you will have to embed the form in your website so you can have a custom icon you want by adding a piece of HTML code between the head tag of your webpage source code where the form is embedded.
