My form will not submit and is stuck on "Please wait". I have had a successful submission before, but since I have edited it it's just stuck. I added a Paypal widget and then ended up deleting it so it is no longer integrated. I use the online form and just want to know how to make it up and running again so I can send my form out again. Thanks!

Could you try reverting the form to the earlier version from the form revision history?

I cannot do that because I changed almost all of the form content since then (on purpose). What are my other options since the form will not submit?

From my end, the submit button doesn't get stuck on "Please wait...", but I found a couple of issues with your form.
1. There's a TEXT FIELD in your form that causes form validation to stop working.
2. Your SINGLE CHOICE FIELDS (radio buttons) are required but none of them have any options.
This means that when someone submits your form, even if they tick/check the radio buttons, they end up selecting nothing since your radio buttons don't have any options to start with. As an example, this is what I end up seeing when I try submitting your form:
To fix these issues, please do the following:
1. Change the field name in the TEXT FIELD under the BOOKING PROCESS & PAYMENT TERMS field. From {first}, change it to {seniorsName}, which is the correct unique name of the NAME field.
2. Then, check and review all of the SINGLE CHOICE FIELDS in your form and add an option to each of them. Take the first one as an example: ACKNOWLEDGMENT OF BUSINESS & CONTRACT FORM
That label should be the option for that field. You can follow this short screencast below:
