Hi there. Why are form data out of order? Example, a patient will fill out the form as I have it organized. Reviewing the form online, it's in right order. Then when we take it to PDF, it's all sorts of scrambled. Please advise.
I am uploading a screenshot of the "incorrect order" but I can't send two screenshots to show you correct order. It goes:
Have you already set up an appointment>?
Which office?
First?
Middle?
Last?
Called?
(those fields are abridged)

Sorry for the inconvenience this may have caused you. Yesterday we had an issue with PDF Editor which caused the wrong order of form fields inside of the PDF report. The root cause of this issue is fixed by our developers.
Please download a new PDF report to see if the issue is resolved. If the issue is not resolved, please re-arrange the form fields or create a new PDF document.

https://hipaa.jotform.com/rbrice1981/npih ", we see that everything is in
the right order. Then we click PDF to download the PDF and it's all out of
order.
I have exhausted your methods outlined above to fix this and it's
unresolved. Please solve the issue. Thanks

Our apologies for the inconvenience caused. I was checking your form and its PDF template. It seems that PDF template was not generated correctly. Probably it must be updated for your form. To do so, please create new PDF document and then you can delete old one, new template should capture all form fields in correct order: How-to-Customize-PDF-Submissions-Report
