Link forms to email account

I have been back on forth on a previous thread (that has now been removed by Jotform without any notification or resolution to my issue) about obtaining information needed on 2 forms that were previously used through and email address that I can no longer access. I have sent everything that Jotform has asked for (proof of ownership of these 2 forms, pictures, videos, etc.) and have been told over and over again that I cannot access these forms.

My church has a health fair coming up and I need the information from these 2 forms (so I know who has registered for what).

Could someone please be of some assistance to this matter?

Thanks for reaching out to Jotform Support. I was able to locate the other account using the form URL you shared above. The current owner of the forms is h***rsvill****lthh**@g*ail.com. If you recognize the email, please log in to the correct account to access the forms. You can also reset your password in case it's forgotten. let me show you how:

1. Open the Login page and click Forgot Password.
2. Enter your username or email then click Send Reset Instructions.
3. Check your email for the password reset email. Click the Reset Password button provided in the email.
4. Enter your new password then click the Create a New Password button.
