Thanks for reaching out to Jotform Support. If you are using a Smart PDF form, please note that it is not possible to map the fields to the PDF document. Please explain the issue a little bit more with the Form URL and we'll do a more in-depth investigation and work on finding a solution.

Can you confirm where you're trying to print the form from? If you're trying to download or print the fillable PDF of the form, I'm afraid fillable PDF doesn't also support widgets, including the Form Calculation and the Configurable List widget your form has. If not, it'll help if you can confirm which widget you're referring to and share a screenshot of the page where the widget in question is supposed to be displayed. Let me walk you through how to share images and screenshots with us:

1. Click the View this thread button on the email you'll receive for this/my reply.
2. Click on the Add Image icon on the toolbar in the Your Answer section.
3. Paste a link to an image or upload a screenshot from your computer.
4. Click on the Add button on the modal.
5. Click the Post Answer button to send your reply.

Upon checking the PDF Editor of your form, I only see a fillable PDF version of your form, and no Submission PDF Template. With that said, I'm assuming that you were referring to the fillable PDF version and why the Fill in the blank widget isn't showing up. Unfortunately, as mentioned by my colleague Christian, the element is not supported in the fillable PDF version. You can check this guide for the whole list of supported elements.
