Creating a question with yes/ no option with additional information

I am trying to create a form where I am asking some yes/ no question. And if the patient says yes, the I will ask him to explain his yes.

For example:

Do you jog daily?

Yes[]    No[]

If No, how frequently, per week:___days

The options are multiple choice or yes/ no. Is there some way that these can be rubbed together.
