How to Change Displayed Page Based on Conditions

    Hi, this form is for collection of personal info. However, when collecting medical information, we want to stop the person from continuing filling the form if certain conditions are detected (e.g., having medical history of heart diseases). I am thinking if I can set up a page which ends the form and tells the person to contact staff, only show it when the conditions are detected, and not showing it if the conditions are not detected (i.e., if they have no medical history of heart diseases, they can continue filling in the form without seeing the page aforementioned). Can I do this action in jotform? Thanks!

    Hi hkprogram,

    Thanks for reaching out to Jotform Support. It's possible in Jotform to achieve this by setting up the correct conditions. Let me show you how:

    Let's first add a Single Choice element to your form and customize it:

    1. In Form Builder, click on the Add Form Element menu on the left side of the page.

    2. Under the Basic tab, scroll down to Single Choice, and drag and drop a Single Choice element to your form.

    3. Click on the title of the Single Choice element and change its name and the choices.

    1. In Form Builderin the orange navigation bar at the top of the page, click on Settings.

    2. In the menu on the left side of the page, click on Conditions.

    3. Click on Add Condition and then click on Skip To/Hide a Page.

    This will hide the second page if the "No" option is selected, directing users to the page following the second page. Check out my screencast below to see how it works:
