I have configured everything. But the unique ID does not display in the autoresponder e-mail and subject line. Although, it does show up in the notification e-mail.

This is odd, but I can not make it work on a cloned form as well. And other fields do not show up too. If you do not mind starting autoresponder fresh, please try: Setting-up-an-Autoresponder-Email
I am also doing some more tests in order to find out what is causing this issue. I will update you via this thread.

I was able to find where problem appears. Please exclude unique id widget from conditional statement that hides multiple fields:
On my test form this helped to resolve the issue for autoresponder email. Please let us know how it goes for you.
