Forms locked due to HIPAA

    I recentle get locked a formulary i need customer support

    Hi consultoriointegralhispano,

    Thanks for reaching out to Jotform Support. It appears two of your forms are locked due to HIPAA. If you are not a HIPAA-covered entity, please go to the Data page on the Account Settings. A HIPAA modal will appear and then, please select the "No, I'm not a HIPAA-covered entity" option. Let me show you how:

    1. Click your Profile Avatar icon on the top right corner of the Jotform website.

    This should mark your account as not a HIPAA-covered entity. Can you give it a try and see how it goes? If you cannot see the HIPAA modal when you visit the Data page, please try to log out of your account and log back in.

    Hi Thanks for your answer but modal is not appearing, and im not a HIPAA institution, could you help me please

    Could you try using another browser or in incognito? If the popup modal doesn't appear on the Data page of your account, please let us know.

    Give it a try and let us know how it goes.

    I tried both, incognito mode and new browser and nothing, no modal, any chance that you can change it please.

    I finally was able to do the update respect to my HIPPA forms restarting the pc but formularies still locked. any sugestions.

    Hi consultoriointegralhispano,

    Once done, the HIPAA requirement will be removed and you can easily enable your form there are two methods to enable your form. Let me show you how:

    Method 1: My Forms

    1. Go to My Forms, select the form you intend to enable,
    2. Click More, then change the form’s status by clicking either Enable.

    Method 2: Form Settings

    1. On Form Builder, in the orange navigation bar at the top of the page, click on Settings.
    2. Under the Form Setting menu on the left side of the page, click on Show More.
    3. Then, set the Form Status to Enable.
