Hello,
I have a form that I have built, and I want the form sent to only one specific email account according to the answer on the first question (Miss or Teen drop down box). If "Miss" is chosen, I want the form sent to one specific email. The same with "Teen". However, I'm now getting two emails for each entry: One where the sender shows as JotForm, and the 2nd shows the sender as the person who filled out the form. Why am I getting two emails?
Please help!

I have checked out your form and it seems that you got the conditions mostly right but it seems that you use the same notification email in both of your conditions. This is why when either option is chosen you get two same emails. I have cloned and tested your form.
If the following changes are made you should get only one email per choice.
If the option "teen" is chosen:
if the option "Miss" is chosen:
Notice that "SEND" part is different in each condition. You can, of course, assign different email addresses to the "EMAIL" part as you wish.
