Hi John,
I checked, and it seems that the image you have shown is using the Image Checkboxes widget:
The smiley face images were manually created by the form owner and added the image URLs in the widget:
You can check our Image Checkboxes widget Demo form.
Let us know if you have any other questions.

Hi John,
Can you explain a bit more about the issue, so I can get a better idea of what you are trying to achieve? Will the chart be visible on the form or only on the downloaded report through the submission table?
Once we hear back from you, we'll be able to help you with this.

Hi John,
If you'd like to use the Emoji Slider, your form must be in Card Form layout. Let me show you how to change the layout of your form:
- In Form Builder, click on the Paint Roller icon on the right side of the page to open Form Designer.
- Click on the Layout tab and select Card Form. That's it.
- While in Form Builder, click on the Add Form Element menu on the left side of the page.
- Under the Basic tab, scroll down to Survey Elements.
- Drag and drop Emoji Slider to your form.
To view the result of each user, you will need to create a report. To avoid confusion, I created a new thread and helped you with that here.
Reach out again if you need any other help.
