I want to send a permission slip for parents to sign for 100 kids. So they each need to sign their own version of the document and fill it all out. And I want to make it digital so it is easy. Can I do this with you?

Thanks for reaching out to Jotform Support. Yes, you can. Jotform is an online form builder that provides a number of tools and 10,000+ customized templates. You can check this check our Permission Slip template to see if it fits your needs. In addition, dragging and dropping features or elements makes it really easy to create forms. I'll help you do it:
- On your My Forms page, click on Create Form on the top-left side of the page.
- Choose Start From Scratch, and select Classic Form to create an empty form, or Use Template to choose among our 10,000+ ready-to-go templates (Start From Scratch was used in this example).
- Now, in Form Builder, click on the Add Form Element menu on the left side of the page to see the complete list of elements and widgets.
- Under the Basic, Payments, or Widgets tabs, scroll down the list, and then drag and drop the fields onto your form. And that’s it.

Hi Kevin,
Yes, that is possible. Jotform makes it easy to integrate your PDF files using Smart PDF Forms. Let me walk you through it:
- Click the Create Form button on your My Forms page.
- Choose Import Form and then Import PDF Form.
- Now, upload your PDF file.
- Add the necessary fields and customize them using the Add Form Element menu on the left side of the page.
Sometimes, the fields that are automatically created in the PDF file will be in the wrong place. If that happens, you'll have to move and resize the fields. Check out the screencast below to see how to do that:
- Finally, click on the Wand icon below the Adobe Sign Widget to open Widget Settings.
- Now, in the Adobe Sign section, on the right side of the page, click on Authenticate to authorize the widget.
That's it. The final preview will look like the one below:
You can also create a Signable Document. Let me show you how:
- Go to your My Documents page and click on Create Sign Document.
- Now, you can upload your own document, or you can use the Demo Document to test it out.
- Then, you can add more documents, or you can just continue by clicking on the Create Signable Document button.
- That will redirect you to the Sign Builder page.
After creating your document, you can let the system detect fields automatically by clicking on the Detect Fields button, or you can manually match your fields with elements by dismissing the pop-up message:
