I sent the attached email message to address this issue Nonetheless you still decided to lock me out of my two forms. Please explain why you feel we are a HIPPA covered entity?
Per your attached message below, please be advised where are not a HIPAA Covered entity.
For example: ‘our research tells us that these entities will fall under a HIPAA Covered Entity.
- Health plans: Private entities like health insurers and managed care organizations, as well as government organizations like Medicare, Medicaid, and the Veterans Health Administration
- Health care providers: Doctors, clinics, psychologists, dentists, chiropractors, nursing homes, and pharmacies
- Health care clearinghouses: Organizations that process nonstandard health information.
In addition our CMS-Center For Medicare Services, research also confirms that we are not a HIPPA Covered Entity. – Please refer to the following link:

Hi Edward,
Thank you for reaching out to Jotform Support. May I know if you received an email stating that you may be a HIPAA-covered entity? If so, if you are not a covered entity under HIPAA, kindly respond to the email by stating that you are not. This way, your forms are not going to be disabled. We need to ensure that the users are not collecting sensitive health information without being covered.
Also, you will need to access your Data Settings and select No, I am not a HIPAA HIPAA-converted entity, so your forms will be reactivated. You will need to be logged in to your account. Let me show you how:
1. In My Forms Dashboard, click on your Profile/Avatar in the upper right corner of the page and click on Settings.
2. Select Data from the left panel menu.
3. Click the Go Back text when the pop-up message shows up
4. Finally, click on NO, I'm not a HIPAA-covered entity.
Once you are done, please log out of your account and log back in, then see if the HIPAA message still pops out.
