What would stop the Jotform from attaching the correct PDF template (as created in PDF Editor)?
NOTE: this was working up until today.
What is being sent is the stripped back/old version and not the pretty version I have setup in PDF Editor:

We have added an option to specify PDF editor version for every form. Probably this setting was changed for your form, please set 'Use NEW PDF Editor' to YES via form settings. Here is a screenshot:
