How do I set up my form so that each check box (they are named first choice and second choice) can have only one box checked? I have attached a screen shot below.
Our volunteers have the option of choosing first and second choice jobs but they can currently check as many as they want right now. I would like to limit that to one check per choice.

I have checked your form and you're using input table using Checkbox as Input type.
Unfortunately, we can only require the field to have an input. There is no option to check for only a single choice answer.
The only way to have a single selection if you change your Input type to Radio button.
However, seeing that you have already submissions in your form. I would suggest creating a new input table and hiding the old one to prevent the submission data from being deleted.
Do note that changing your input table will delete the submission data.
