This is URGENT! Why is my account suspended? And what can I do to retrieve my account ASAP?

Thanks for reaching out to Jotform Support. I checked your account and found that the account has been suspended due to the form asking for username and password. Asking login credential is against the Terms of Use.
We have removed the offending field and we have reactivated your account. You can try logging into your account and check if the issue still remains.Reach out again if there’s anything else we can do for you.
