Hi Vanya,

You can use the Single Choice element for your questions and answers and the Form Calculation widget to calculate the answer values and give a rating. It's very easy to do. Let me show you how:

1. While in Form Builder, click on the Add Form Element menu on the left side of the page.

2. Under the Basic tab, scroll down the list and choose the Single Choice field.

3. Now, just drag and drop it to your form.

4. Click the text to edit the label, or you can go to the Field Label section under General tab and Options section for the choices under Options tab by clicking the Gear icon.

5. In Options tab, enable the Calculation values and set the values for each choice to your preference.

6. After that, in Add Form Element menu, select the Form Calculation widget and drag and drop it to your form.

7. Click the text to edit the label, or you can go to the Field Label section under Advanced tab by clicking the Gear icon.

8. Finally, click the Wand icon to modify the formula by adding fields, perform the Arithmetic operations and then save.
