Where did my Jot Form go?

Hello!

I created a jot form that is still active - but is not longer in my forms or account to clone, edit, or anything. There is just no history of ever having the Jot Form - yet her is the link: Business Insurance Questionnaire (jotform.com)

How do I get this template added back to my account?

Thank you for reaching out to Jotform Support. The form is in your account, but it is located in the My Teams workspace. Let me show you how:

1. In My Forms Dashboard, go to My Teams.
2. Click the Teams name, and you will find the form in there.

Give it a try and let us know ho it goes.
