Approval flow returns an email that says undefined.

I am having issues with the VTO Survey. It is saying the action is undefined for action required...

Thanks for your patience, we appreciate it. I've run a few tests on the clone of your form, and the issue keeps on persisting. So I redo the approval flow and everything is working as expected now. Must've been a glitch in the system, but here's how you can redo the approval flow:

• In Form Builder, in the top-orange navigation bar, go to Settings.
• Click on the Approval Flows tab in the left panel.
• Hover your mouse on your approval flow, and click the pencil icon that'll appear.
• Click on the fields of your flow and click the trash icon that'll appear beside it.

• Now, add your approval flow again, following the format below:

It is still saying undefined in the email where I should be selecting or approve or deny.

I need to be able to reassign this selection to one of our managers in the company. The same I could do for the Employee Payroll form I have in our forms...

Please let me know if I am doing something wrong.

I cloned your form and tested it to see if I could replicate the issue, but everything was working properly, I was able to receive the approval email. Check out the screenshot below to see my results:

Did you redo the approval flow as mention by my colleague above, which might solve your issue? Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.

I actually created a new VTO form to try and see if I needed to start over and it is still showing undefined for action required in the emails.
