Incident report or request form

    I have created this incident form that provides fields for data entry based on one of two selections.


    I would like to select "Edit this submission" and be able to

    • write something back so the submitter gets an updated form by email
    • Provide a status such as "Open', "closed" , "WIP", etc so the submitter gets and updated form by email

    But I do not the submitter to be able to enter anything in these fields (that I enter information into) but only see what i send.

    Hi rumijas,

    Thanks for reaching out to us for help. What you want to do is possible. Suppose you already added the additional fields on your Incident Report form where you can enter your comments and select a status.

    You can use the Get Form Page widget and a logical condition to show those fields only when you access your form in edit mode. Let me show you how:

    1. In Form Builder, click on the Add Form Elements menu on the left side of the page.
    2. Under the Widgets tab, search for the Get Form Page URL and drag and drop it into the form.

    Once you've added the widget to your form, we will set up the logical condition. Let me show you how:

    1. Next, in the orange navigation bar at the top of the page, click on Settings.
    2. Click on Conditions on the left of the page.
    3. Now, click on Show/Hide Field. If there is at least one existing condition, click on Add Condition first.

    4. Set the condition similar to the condition below:

    5. Click on Save.

    And that's it. Check out the screenshots below to see the result:

    Now, when you edit a submission, the comment and status fields will show on the form but it won't be shown to your respondents. It should give the same result when you edit submissions using the Jotform Tables' Edit Entry feature.
