Is there a better option , for a form than this ?

Can you tell us which embed code did you use, please? Which-Form-Embed-Code-Should-I-Use
Also, is your website build on a specific website building platform?
To be honest, it would be difficult for us to troubleshoot the issue without getting to check the website itself.
Would it be possible for you to create a test website that you will be able to give us permission to view/access?

Please try to embed the form using the iFrame method. This will prevent script conflicts between the form and the webpage. Here's a guide: Getting-the-Form-iFrame-Code.
