Cancel and refund the account that is no longer active.

    Hello,

    I was charged $288 from Jotform recently. We had an account with Jotform in the past. Our company went out of business at the beginning of 2023. I thought my associate cancelled all of our subscriptions.

    Can you please look into this and cancel the account. It would be under either rkaufman@executivesinmotion.com or amitich@executivesinmotion.com. The account I am sending you this request under (rkaufman@executivesinmotion.com) is at the Starter Level under for which there is no payment.

    I would also like to request a refund of the $288 as well. Please let me know if this is possible.

    Regarding the emails above for the accounts, these emails are also no longer active. If you would please forward all correspondence to my person email bobbykauf@comcast.net, I would appreciate it.

    Hi Robert,

    Thanks for reaching out to Jotform Support. Unfortunately, we don't have the option to send our responses to another email address. It will automatically be sent to the email address associated with the account you're using. In addition, I was able to find the paid account, which is under the email address amitich@executivesinmotion.com and is currently on Bronze Yearly. But for us to verify that this is your account, could you please provide us with the information below that was used when signing up for the subscription:

    • The type of Credit Card used while upgrading the account:
    • The last four digits of the Credit Card:
    • The expiration date on the Credit Card:
    • Amount of the last charge:

    Once we hear back from you, we'll be able to move forward and help you cancel the subscription.
