O formulário não é enviado em dispositivos móveis e computadores

Quando eu clico no botão de enviar o formulário, o formulário não envia. Não é erro, pois não apresenta nenhuma mensagem. Mas quando eu clico no botão de enviar aparece uma mensagem do google, como eu vou mandar no vídeo (não consegui enviar por aqui) e na imagem anexada, são 2 smartphones diferentes testados e também no meu deskop. I've gone ahead and cleared your form caches to refresh your form. Can you please try to submit your form again if it makes any difference? Also, If the issue persists, can you please share with us a short screen recording using Loom for us to understand what's going on?
