Form are not sending submission to Google Sheets

Our Volunteer Hour Submission form is integrated to Google Sheets and also has an approval flow. At one time the approval would update on the Google Sheet but that stopped working. Disconnecting and reconnecting the integration did not fix the issue. I did not build the form initially so I do not know what made it work. Thoughts? Thanks.

Thanks for reaching out to Jotform Support. I believe you are referring to this form. Please note, any changed made to the form and its integrated sheet will break the integration. When this happens, you will need to disconnect and reconnect the integration and ensured no changes are made to the form and integrated sheet.

In the meantime, I have cleared your form cached and tried to push submissions to your spreadsheet, but it looks like there is no new submission in this form. If this is not the form you are referring to, can you share with us the form URL so we can help you push the submissions?

5953324855228889044 is a sample submission ID but the issue has persisted for dozens of submissions to the form.
