    Is the form hosted on a site that has a static IP address?

    Hi esmileconsult,

    Thanks for reaching out to Jotform Support. Jotform is hosted in several data centers. Our submission servers do not have static IP addresses and the IPs are dynamic and will change over time. Static IP addresses are only available for our Enterprise Users.
