    Hi Krista,

    Thanks for reaching out to Jotform Support. I reviewed your account and discovered that it was suspended because you have a form named Credit Card Authorization Form to collect credit card information, which violates our Terms of Use. We can only reactivate your account if you allow us to remove the credit card fields in the form. 

    Due to our PCI certification, we are required to deny the collection of credit card details on regular forms. However, we have other PCI-compliant ways to do this. All you need to do is choose any of the payment processors available on our platform in order to integrate into your forms. You will still not be allowed to collect credit card information directly.

    If all you want to do is collect your customers' payment information and bill them later, you can use a function called "Payment Authorization". To learn more, see our guide on How to Enable Payment Authorization.

    Once we hear back from you, we'll be able to help you with this.

    We have deleted the offending fields from the form and reinstated your account. You should be able to log in now.
