Help me create an entry form for a competition

    Hi

    Would you please help me design an entry form for a competition.

    In short the competition has the following

    • Each team consists of 2 people
    • within each team there are 2 separate jobs - Lets say Job A & Job B
    • Each individual can be either an A or B depending on their skill set
    • Each individual can have a handicap rating separately for A & for B
    • Each Event class a competitor can enter up to 5 times either as an A or a B. Therefore if they enter as an “A” skilled competitor then they must find a “B” skilled partner and vice versa.
    • each event class has a rating such as 6, 8, 10, 12
    • the calculation of both A & B competitors cannot exceed the class number
    • for example in class 10, A=6 & B=5 they total 11 - so therefore dont qualify for that event.

    So ideally i want a form to do the following:

    1. Have option to Enter separate classes with different team members.
    2. Class 4,6,8,10, 12 for example
    3. enter each class up to 5 times
    4. out of the 5 times you can choose if you are doing Job A or B
    5. when you choose your job then your Handicap is populated and when you choose your partner then their handicap is also populated.
    6. The total team calculation is then added together.

    is it possible to have the handicaps uploaded from an existing spreadsheet so that when a person is selected then this would populate?

    I would like this to become a table so that each members entries are comprised in a table format

    Hello Nathan,

    Jotform is an online form builder that provides a number of tools and 10,000+ customized templates. Dragging and dropping features or elements make it really easy to create forms. I'll help you do it:

    1. In Form Builder, go to your My Forms page.

    2. On the left side of the page, click on Create Form.

    3. Click on the Add Form Element menu on the left side of the page.

    4. Under the Basic tab and Widget tab scroll down the list, choose the options that you want, and then drag and drop them onto your form.

    • We used the Gift Registry Widget to list the classes limited to five submissions each.

    • We used the Form Calculation Widget and set up the formula as follows:

    • Add Heading Element, Multiple Choice (right click then choose shrink to make these 2 Multiple Choice listed in two columns), and Form Calculation Widget for each class.

    • Add Calculation Values to Job A and Job B, then set up the formula to Form Calculation Widhet labeled "Class Rating" and combine the Job A and Job B values.

    • Please repeat the steps above for the exceeding classes. ( Class 6,8,10 and 12).

    5. Set up the conditions below so that whenever a Class is selected from the Gift Registry, the fields intended for the selected classes will appear.

    I'm not sure about your requirements for creating the handicap questions. If it does not require calculating, you can simply add an element that fits the type of data you need for this question. As for your other question, I've moved them to new separate threads. You can check them below:
