We frequently need to use yes/no questions, and if the answer if yes then we want more details and possibly a file upload. In your system we can achiave this, but it requires lots of conditional logic and space.
I'd like to suggest a special field type calles Yes/No - using radio buttons in-line in column mode and if yes is selected then (based on settings) additional instructions, long answer response and an upload file micro button be added dynamically beneath the field

Hi Peter,
Thanks for reaching out to Jotform Support. There is a workaround in which you can use the Input Table with Yes and No radio buttons to display fields based on the input. Let me show you how:
- In Form Builder, add a Input Table and configure the rows and column.
- Next, add a Form Calculation widget and add the Input Table in the formula, setting it as a string.
- Add a File Upload and Long Text field.
- Finally, create a Show/Hide field condition and set it as follows.
