Smart PDF Form: How to scan the original form and add it as a background

    Additionally, I would have liked to scan the second page of the physical form and use it as the background of the extra info. Can't seem to figure out that one.

    Hi Ray,

    Can you explain a bit more about the issue, so I can get a better idea of what's happening? Are you trying to add an image to the PDF version of your form? Can you clarify what you mean by scanning the second page of the physical form to use as the background of the extra info?

    Thank Renee!

    I used a single page to create the "smart PDF" and the back of the form was not included. I would now like to add it as the second page background.

    The original form (similar to an application) calls for Yes/No questions to have an explanation when affirmative. With JotForm I've set up conditional fields that get filled only when the answer to the previous question is "Yes" and I've set the PDF to display this extra information on a second page as there is no room on the primary main page. These notes have no heading or visual cue as to which question they are explaining as that page has a blank background. I would like to add visual lines or subdued headings etc to for clarity.

    Hi Ray,

    Unfortunately, we can't edit the original PDF you have uploaded in the Form Builder. As a workaround, we can generate a new PDF document that will include the added form fields with the corresponding question. It's really easy to do. Let me show you how:

    1.  In PDF Editor, click the New PDF button. That's it.

    Doing this, you will be able to see which form field was answered.

    Alternatively, you can reupload your original PDF with a designated spot for the additional details. Let me show you how to do it:

    1. In Form Builder, in the blue navigation bar at the top of the page, click on Upload.
    2. Click the Update PDF and choose Re-Upload PDF.

    Selecting “Start Over” will delete your previous form submissions and you will have to map the form fields again. We also have a guide here that you can check to learn more about How to Update Your Original PDF in Smart PDF Forms.
