Can I please have my account reinstated? I was in the middle of creating a form and it looks like my account is now suspended.

Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named "Website Questionnaire Form" that’s breaking our Terms of Use by collecting login credentials.

I've deleted the offending fields and reactivated your account. You should be able to log in now.
