Hi Conally,
Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named Payment Authorization Form that’s breaking our Terms of Use by collecting credit card information in plain text fields.
In line with our PCI certification, we have to deny the collection of credit card details on regular forms. But, we have other PCI-compliant ways you can do this. All you need to do is choose one of the payment processors available on our platform and integrate it into your form. You still won't be able to collect credit card details directly, though. If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of our Payment Authorization feature.
If you’ll agree to follow our Terms of Use and let us delete the offending fields mentioned above, we can lift the suspension on your account.
Once we hear back from you, we'll be able to move forward with this.

Yes, please delete it. I only have three forms I want to use.

Hi Conally,
I have deleted the offending fields and lift the suspension on your account. You should be able to log in now. If you encounter any issue with logging in, please try to reset your password. It's easy to do this, let me show you how:
1. Go to the Jotform Main Page and click on Login.
2. On the login screen, click on Forgot Password.
3. Enter your email address and then click on Send Reset Instructions.
4. You'll get an email with instructions about how to reset your password. In the email, click on Reset Password.
5. Enter your new password, enter it again to verify it, and then click on Create a New Password.
Your new password is now set and you can use it to log into your Jotform account.
