Create a form out of a PDF document.

    I have a consent form I already use and would like to upload that one.

    Is that possible or do I need to use the templates that the website has? I worked on it for hours and I would like to use it.

    Hi Iris,

    Thanks for reaching out to Jotform Support. Your question is a bit unclear, but if you are trying to use a PDF file that you already have as a form then that is possible. You can convert a PDF file to a form using Smart PDF Form. Keep in mind that the Smart PDF form can only detect fillable fields in your PDF document. Let me walk you through it:

    • In My Forms page, click the Create Form button. 
    • Now, choose Import Form and then Import PDF Form.
    • Next, upload your PDF file.  Create a form out of a PDF document Screenshot 20
    • Add the necessary fields and customize them using the Add Form Element menu on the left side of the page.
