    Hi Patti,

Thanks for reaching out to Jotform Support. If you are trying to create an approval flow for your Survey Form, and attach PDF files to each response if a Survey is Approved or Not, yes this is possible. Let me walk you through the steps:

    1. In Form Builder, click on Settings in the orange navigation bar at the top of the page.
    2. Now, click on Approval Flows in the left panel. Then click on + Create Approval Flow button. This will route you to Approval Workflow page with a simple approval flow.

    3. Next, in Approval Workflow page click on either Approve or Deny branch and click on the Envelope Icon.
    4. Finally, on the pop-up window, go to Advanced tab and Drag/Upload the PDF file that you wish to attach when the Survey is Approved/Denied.

That's it. If that’s not exactly what you’re looking for, can you explain a bit more so I can help you better?

Give it a try and let us know if you have any other questions.

Hi. What I need is a form that has an approval or not approved. However what they have to approve is in a PDF file. So they would need to read the PDF and then select their choice. Does that make sense?

    Thanks.

    Hi Patti,

You can use the PDF Embedder widget to add a PDF in your form that you users can read. It is very easy to add. Let me show you how:

    1. While in Form Builder, click on the Add Form Element menu on the left side of the page.

    2. Under the Widgets Tab, scroll down the list and choose the PDF Embedder.

    3. Now, just drag and drop it to your form.

    4. Click the Wand Icon and Upload the PDF Document that you have.

After this, we can create a condition that will hide the Submit button and only show it after they have finished reading the PDF and approved its content. It's easy to do. Let me walk you through it:

    1. In Form Builder, click on the Add Form Element menu on the left side of the page.

    2. Under the Basic Tab, add a Single Choice field to your form.

    3. In the orange navigation bar at the top of the screen, click on Settings.

    4. Click on Conditions on the left side of the screen.

    5. Select the Show/Hide Condition.

    6. Create a condition that hides the Submit button.

    7. Click Save.

That's it. Doing this will make your users read the PDF first and click the Approve choice before they can submit the form.

    Hi Ronald.
Thank you for the information. Is there a way for the PDF to just be a link
they have to click to view?
    Thanks.

    Hi Patti,

Yes, you can have the link of your PDF to be embedded in your Form. It is very easy to do. Let me show you how:

    1. In Form Builder, click on the Add Form Element menu on the left side of the page.
    2. Under the Basics Tab, scroll down the list and choose the Paragraph Field.
    3. Click the Pencil Icon and add the text in the Paragraph Field.
    4. Click the Insert link icon and add the URL of your PDF.
    5. Click the Check Icon to save and you're done.

That's it. Once your users click on the text, they will be redirected to the URL that you've added. Once they have read it, they can continue with the previous flow by clicking the option that they have read it, and it will show the submit button.

Thanks. I have it figured out now.

But I have another question - is it possible to have a response automatically show (in my emails) who replied without them having to fill in any information?
