Hi Kanak,
Thanks for reaching out to Jotform Support. If you can share the link to your form, I can check it out and see what’s going on. If you can take a screenshot of what’s happening, that will help us to better understand what’s going on. We have a guide about how to do that here. You can check this guide for more information.
Once we hear back from you, we'll be able to move forward with a solution.

here is the form link
https://form.jotform.com/241584624505053
There is field name called Customer name and would like to map to SF Account name with ID.
so basically when user type customer name = ABC and submit data
it will submit in SF side Customername = 123456 (ID)
so i'm looking for is there anyway i can build look up data table and when user try to type customer name it prepopulates name and when submit data it submit ID.

Hi Kanak,
If I understand correctly, you want to create a record, that has a relationship to the account in your Salesforce. You can integrate Salesforce and create two actions to achieve that workflow. I cloned your form, and integrated Salesforce into it for a demonstration. Check out the screencast below:
As you can see, I was able to create a record, that has a relationship to an account in Salesforce. Let me show you how to do it:
1. In Form Builder, click on Settings in the orange navigation bar at the top of the screen, and in the menu that opens on the left side of the page, click on Integrations.
2. Search for Salesforce, and click on Salesforce.3. Authenticate your Salesforce account.
4. Click on Find Existing Record, and choose Account in the dropdown.
5. Map Account Name to Customer Name.6. Toggle on the switch for Create a Record.
7. Map Account Name to Customer Name.
8. Click on Save Action.9. Click on Add New Action.
10. Choose Create a Record, and select Case in the dropdown.
11. Map the required fields, and map Account ID to "Step 1. Find Account Record."12. Click on Save Action.
13. Click on Save Integration.
14. Click on Finish.
That's it. Let us know if you have any other questions.

kanak1983Replied on June 27, 2024 at 4:42 PMThank you it works exactly how i wanted! but we wanted to show auto populate name for customer name when user enter data in customer name field which come from SF accounts. is it possible?
i could able to do in below screen shot with autocomplete widget but then i have to enter individual items. it would be nice if i can upload spreadsheet or use table to link this column to auto populate name when user type. make sense?
right now if they enter exact name then it will linked up with existing record otherwise send empty value.
