I have a previous employee that had a membership with Jotforms and they are no longer with St. Paul's but the charges continue through their credit card. I have inactivated their card but need to have this membership cancelled.

Please confirm the details below for us to cancel the subscription:
- Name on the card
- The last charge amount and date
- The last four digits of the card
- The expiration date of the card
Once we hear back from you, we'll be able to help you with this.
